We offer plans from Security Life/Prime Star Dental, United Healthcare - Golden Rule, Anthem, and Medical Mutual.  Consumers looking for comprehensive dental insurance with an optional vision rider can find it here.

You can choose from several tiers depending on the level of coverage you desire.  Most companies offer at least two plans to choose from. Carriers such as Anthem, UHC, and Medical Mutual can also bundle health insurance into your coverage.

Eligibility, Deductibles and Waiting Periods

Dental and Vision InsuranceIndividuals 18 years of age or older and their dependents are eligible for coverage. Senior citizens may also apply for coverage.

Routine preventive services (cleanings), basic services (fillings), major services (such as oral surgery/crowns) and orthodontic services will be covered ─ depending on the plan selected.* An optional rider allows you to increase your maximum annual benefits for a monthly fee. In addition, an optional vision rider will provide 85% coverage for vision exams and 50% benefits for lenses and frames. Contact lenses are covered 50% if purchased in lieu of glasses.
